If you'd like to participate in any CSRT Program this season, you will need to download and complete the following forms:

• CSRT Release of Liability
• CSRT Application and Release
• Mt. Hood Meadows Competition Liability Form
• Ski Bowl Release Form
• Volunteer Deposit Agreement
• Athletes Code of Conduct
• Parents Code of Conduct

Once the forms are printed and filled out, send them to CSRT at PO Box 1246, Hood River, OR 97031. Questions? Email Us.

USSA Membership

USSA Membership mandatory for J3 Competitors, the Competition Team and recommended for the Training Team.

• USSA Membership Application for Training and Competition Teams - ($125 for J3 Racers and $60 for J4 and younger. $25 late fee will apply for J3 competitors if sent in later than Oct. 15, 2008. No late fee for J4 and younger.) Please submit USSA License directly to USSA - fax and address is listed on form.
