If you'd like to participate in any CSRT Program this season, you will need to download and complete the following forms:

• CSRT Release of Liability
• CSRT Membership Application
• CSMR (Cooper Spur Mountain Resort) Competition Liability Form
• Mt. Hood Meadows Competition Liability Form
• Mitey Mite Council Release
• Volunteer Deposit Agreement
• Athletes Code of Conduct
• Parents Code of Conduct
• USSA Membership Application for Red Team J5 & Older
